Life Insurance Appraisal

  • A qualified appraisal for gifts of life insurance to charity (only for policies with a fair market value greater than $5000)
     
  • We will provide you with a sample letter to help obtain your required information from the insurance company.
     
  • You must provide

    • The insurance contract – first five pages including owner and beneficiary information
       
    • Form 712 Part II completed and signed by the insurance company
       
    • Adjusted cost basis
       
    • Current policy values – most recent policy statement or in-force illustration
       
  • You will receive

    • Form 8283 to support the deduction.
